Graduate Program Preapplication Form

This is a preliminary acceptance application for graduate study in the Department of Chemistry and Biochemisty at Utah State University. Please provide as much of the requested information as possible. Based upon the information you provide, we can inform you of the probability of your acceptance into our graduate program. If you receive a positive initial evaluation, your application fee ($55) will be waived when you submit a full application. Formal acceptance will require submission of official documentation (transcripts, letters of recommendation, GRE/TOEFL or IELTS scores) and a formal application.

Note #1: Since all applications to our department are considered to also be applications for a TA position, English proficiency is a key qualification factor. It is therefore departmental policy to consider payment of the application fee only for students who have earned an undergraduate degree for which the language of instruction was English.

Recruitment for fall 2008 has been closed. We are now accepting applications for Spring/Fall 2009.

Only preapplications listing all information regarding: GPA, GRE, and TOEFL or IELTS (if applicable) will be evaluated. Forms with missing scores will be deleted.
